Join Our Dynamic Team: With the expansion of our organisation, we are looking for a Home Care Packages (HCP) Care Manager (with HCP experience) or Care Coordinator (no HCP experience) in the Greater Brisbane Area. This role is suitable for a person who enjoys human interactions, building new relationships, managing staff members and is passionate about creating a difference in others' lives. Previous Home Care Packages experience is highly desirable but not mandatory. Sensible Care will provide training for Home Care Packages care coordination skills.
Key Responsibilities: Conduct assessments of new clients to understand their individual goals, needs, and preferences. Facilitate the onboarding process for new clients, ensuring a smooth transition into the care program. Develop and implement person-centric care plans in collaboration with clients. Monitor the ongoing effectiveness of care plans, adjusting as needed to address changing client needs. Conduct risk and vulnerability assessments of clients to identify potential safety concerns. Implement strategies for ongoing monitoring of clients' safety and well-being. Coordinate and schedule client and care team member schedules, ensuring optimal service delivery. Collaborate with the team to manage and allocate resources efficiently. Demonstrate a positive attitude towards older individuals, understanding their needs, and upholding their rights and cultural diversity. Act as the first point of call for clients, families, other nominated representatives, healthcare professionals, and other external providers. Communicate effectively with clients, families, and the care team, fostering a supportive and respectful environment. Participate in regular care reviews with clients, families, and other healthcare professionals. Ensure compliance with relevant regulations and standards. Stay informed about updates to industry standards, including the Aged Care Act 1997 and Aged Care Quality Standards. Foster a culturally responsive and inclusive environment within the team. Collaborate with community partners and organisations to improve brand awareness and enhance service delivery. Work closely with care and support staff members to ensure services are safe, individualised, and effective. Contribute to quality and continuous improvement initiatives. Ensure all documentation for clients is accurately maintained and draft reports. Qualifications and Skills: Experience in community care delivery preferable. HCP program knowledge. Strong understanding of consumer-directed care principles. Cultural competence and experience working with diverse communities. Excellent time management and organisational skills. Ability to work autonomously while being a team player. Knowledge of relevant regulations and standards in aged care will be considered favourably. Passion for socially responsible and person-centric care. High level of interpersonal skills and ability to build strong and trustworthy relationships. Demonstrated competence in general computer applications and ability to learn our care management software package. An Australian Driver's Licence. Location: This role requires you to travel around the Greater Brisbane Area to visit clients and prospective clients to conduct consultations, assessments, and reviews.
Hours: Work is between 9 am and 5 pm (with flexibility) and requires a commitment of between 20 to 30 hours per week.
