Care Manager We're seeking a dedicated Care Manager to join our residential aged care facility in Newstead, Victoria.
In this pivotal role, you'll ensure the highest standards of clinical care and quality of life for our residents, leading a dynamic team and fostering a supportive environment.
Summary: As a Care Manager, you will oversee all aspects of clinical care for our residents, ensuring their physical, emotional, and social needs are met.
You'll manage care staff, providing mentorship and guidance, coordinate care plans, and ensure compliance and care standards are met with the overall goal of providing the highest-quality care.
Responsibilities: Oversee daily operations of the care facility, ensuring compliance with all regulatory requirements and company standards.
Develop and implement care plans tailored to the individual needs of residents, coordinating with healthcare professionals and families.
Lead, mentor, and support a team of care staff, providing guidance and professional development opportunities.
Manage budgets, resources, and supplies efficiently while maintaining a focus on cost-effectiveness and quality.
Conduct regular assessments and audits to ensure the facility's services meet the highest standards.
Address and resolve any concerns or complaints from residents, families, or staff in a timely and empathetic manner.
Key Requirements: Registered Nurse with a current AHPRA registration.
Proven experience within a Care Manager position (or similar).
Strong knowledge of Aged Care accreditation standards.
Comprehensive knowledge and experience with ANNAC.
Strong leadership, communication and interpersonal skills.
An outstanding proficiency in care planning, goal setting and evaluation documentation.
Capability to effectively lead and manage a team.
