The Care Finder role is to provide specialist and intensive assistance to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the communities. Kirinari Community Services are working in partnership to deliver the care finder program within the Western NSW region. This position would be covering clients in Cobar, Nyngan, Warren and Condobolin.
The care Finder program is a national program that:
Provides specialist and intensive assistance to help people understand and access aged care and connect with other relevant supports in the community.
Supports improved integration between health, aged care and other systems at the local level.
Connects and engages older people who have significant difficulty accessing aged care services.
Provides assertive outreach by engaging and building rapport with potential clients and local intermediaries, including outreach activities to target groups and target locations.
The target population for the program is people eligible for aged care who:
Are isolated or have no available support person
Have communication barriers, including limited health literacy skills
Have difficulty processing information to make decisions
Are resistant or hesitant to engage with aged care, institutions, or government for any reason
Their safety is at risk, or they may end up in a crisis
Care Finders will work with the target population to communicate and collaborate with participants, carers/families and relevant aged providers both within and external to the health system and support systems to enable integrated care and support.
Who we are looking for.
The successful applicant will be someone who meets the following criteria:
Relevant qualifications (e.g. social work, human services, aged care, community services or health) and/or relevant experience
Demonstrated understanding and working knowledge of the ageing process and related life transitions
Demonstrated understanding of aged care supports and services available in the local and regional communities and process and eligibility requirements to access including the steps of this process that are undertaken via My Aged Care.
A commitment to delivering a person-centred approach that respects and responds to each person's individual needs, preferences, values and life experiences
Well-developed communication and interpersonal skills and ability to engage and communicate effectively with people from a culturally and linguistically diverse (CALD) background and Indigenous communities
A commitment to supporting the needs of people with diverse backgrounds and life experiences whilst treating people with dignity and respect and in a way that values their identity, diverse backgrounds and life experiences
A strong understanding of, and commitment to, cultural safety and trauma informed care
Strong solution focused, problem-solving skills
Strong administrative skills, including an ability to use and accurately enter information into client management systems, databases and/or portals.
Current NSW or National Driver's License
First Aid Certificate or willingness to obtain.
