The Role:
We are looking for compassionate and dedicated professionals to join our team in a blended role as a Care Finder & NDIS Support Coordinator.
This unique dual role offers the opportunity to work across two vital programs: Care Finder Program:
In this role, you will provide specialised assistance to help older adults understand and access aged care supports and connect with relevant community services through the Care Finder program. You will be pivotal in: Guiding individuals through the complexities of the aged care system. Connecting clients with essential community services to enhance their quality of life. Improving outcomes by coordinating between health, aged care, and other local systems. Bridging gaps in understanding, ensuring seamless integration of services.
NDIS Support Coordination:
As a NDIS Support Coordinator, you will deliver individualised support to people living with disabilities, including psychosocial disabilities, helping them maximise the benefits of their NDIS plans. In this role, you will: Assist participants in identifying and achieving their personal goals. Build independence by strengthening formal and informal support networks. Help participants connect with locally available services and make the most of their NDIS plans. Promote inclusivity by contributing to a fair, honest, and respectful workplace that values equal opportunity and safety.
About you:
We are looking for someone who has: An ability to establish and maintain effective relationships with clients and the community A passion to build knowledge of the programs and related service providers Tech savvy with an ability to research and learn about local services A proactive and empathetic approach with a willingness to learn and upskill
