As a Care Facilitator servicing clients in the Bathurst region, you will assist clients and their families to engage in services, enabling them to remain living in their home.
You will empower our clients through individualised care planning and goal-setting focused on our well-living approach.
A typical day will include: Home visiting clients in the community who have been referred for help at homeParticipating in the development, implementation and evaluation of services and programs that meet clients' individual needsDeveloping relationships with families and carers that acknowledge their needsWorking alongside our clients to set individual goals that are realistic and achievableImplementation of best practice and evidence-based approaches for care including working closely with a multidisciplinary teamDocumentation and the use of client record management systemsWhat do we need from you? You will need to be comfortable working on your own in the community, have excellent verbal communication skills and have an empathetic, patient and caring nature.
This role does require you to have:
