About the Role As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services around Mackay& surrounding local suburbs. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit their evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families. Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems.
You will provide a high level of service to ensure our clients and their families feel cared for and supported. This is an exciting opportunity to join a supportive team that are passionate about the work they do and enjoy working collaboratively. This is a permanent full time position, and while you will be based working from home, you will have many avenues to connect and collaborate with the broader team. Some travel to clients' homes will be required for reviews or in an emergency, and travel in your own vehicle is reimbursed.

Key Responsibilities Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholdersEnsure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visitsCollaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering team and Practice LeadsDevelop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the communitySkills & Experience Demonstrated Aged Care, Disability or Nursing case management experience with strong administration skills, and ability to work with individuals, families and/or stakeholdersUnderstanding of Home Care Packages, Commonwealth Home Support Programme and My Aged Care Standards.High level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systemsHigh level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills.Ability to multi-task and work autonomously in a fast paced environmentA current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travel (travel reimbursed).Join one of Australia's leading social purpose organisationsCareer pathways across a national organisationAttractive remuneration package with an optional monthly rostered day off (think 12 extra, long weekends a year!)Up to $15,900 of your annual salary tax free through Salary PackagingSuccessful candidates will be required to clear probity checks including National Criminal History Record Check and complete a statutory declaration.
