About the Role As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Darwin.
Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit their evolving needs.
You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems.
You will provide a high level of service to ensure our clients and their families feel cared for and supported.

Key Responsibilities Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders.Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits.Collaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering team and Practice Leads.Develop strategies and/or broker services which will address the identified needs of clients in consultation with families and communities.Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community.Skills & Experience Time management skills and the ability to prioritise effectively to ensure the high needs of the role are met.Degree or equivalent in allied health, nursing, mental health or a related field as well as previous complex case management experience.Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for individuals and their carers, including people with culturally and linguistically diverse backgrounds.Demonstrated experience in facilitating and maintaining partnerships with service providers and health professionals.Demonstrated experience in the use of brokerage services in a community setting.Highly developed administrative & analytical skills and the ability to apply these skills in case management.High level communication, liaison, negotiation and mediation skills.Successful candidates will be required to clear probity checks including National Criminal History Record Check and complete a statutory declaration.
