This has led to greater collaboration between health professionals. Recent growth has included funding for the establishment of an autism spectrum disorder assessment clinic, funding to support children and families to access the NDIS, and a community-led integrated paediatric assessment clinic in partnership with Berry Street.In this role, you will be working closely with other care coordinators to ensure vulnerable children and families in the Latrobe Valley are provided with intensive short-term care coordination services to link them into appropriate health and social services. As a care coordinator, you will be expected to build a child and families' capacity to self-manage in the future.
