Role Overview Our Far North Queensland office continues to grow and we are looking for an experienced and dedicated Care Coordinator to support our growth. The successful candidate will preferably have a strong background in Community Aged/Disability Care. The role is responsible for service development, coordination, and delivery, including ongoing client assessments and the development of care plans that deliver flexible and personalised in-home support. Direct nursing support to clients is also incorporated within the role, as such a current AHPRA-Nurse Registration is required. The successful applicant will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.
This role requires some flexibility as some weekend supports are likely to be needed, with this time offset as reasonable through the week.
This key role will focus on case management, nursing support, customer service, personal care work where required, and general administration.
