Care Coordinator/ Nurse (Cassowary Coast)Right at Home is one of the largest home care providers in the world, with over 700 offices in eight countries.
We provide a wide range of tailored care services to assist people living independently in their homes and are respected within the community for providing our clients with a high level of service, with our mission being 'to improve the quality of life of those we serve'.Role OverviewOur Far North Queensland office continues to grow, and we are looking for an experienced and dedicated Care Coordinator to support this continued growth in the Cassowary Coast Region.
The successful candidate will preferably have a strong background in Community Aged/Disability Care.
The role is responsible for service development, coordination and delivery, including ongoing client assessments and the development of care plans that deliver flexible and personalised in-home support.
Direct nursing support to clients is also incorporated within the role, as such a current AHPRA-Nurse Registration is required.
The successful applicant will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.This role does require some flexibility as some weekend supports are likely to be required, with this time to be offset as reasonable through the week.This key role will focus on case management, nursing support, customer service, personal care work where required, and general administration.Care Assessments;Government-Compliant Care Management Plans;Holistic outcomes driven Care Planning;Operation of the Care Planning and other management systems;Nursing and personal care supports;Quality assurance.QualificationsMinimum of Diploma in Nursing with a current AHPRA registration;Current First Aid & CPR;Driver's License.Preferred SkillsGood understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);Good understanding of the hospital discharge environment;Subscribe to a culture of teamwork, caring, and quality service;Proven leadership abilities, professionalism, a good reputation & respect for our staff, clients, and their families;Good judgment, quick decision-making, excellent communication, staff management, and administrative skills;Current National Police Clearance.Please note that we review applications and extend offers on a rolling basis, which means we may fill all available spaces for some divisions and locations prior to the application deadline.
