Established in 1991, Danila Dilba Health Service is a community organisation providing comprehensive primary health care to Biluru (Aboriginal) communities in the Yilli Rreung (Greater Darwin) Region of the Northern Territory. We aim to improve the physical, mental, spiritual, cultural, and social wellbeing of our clients through innovative comprehensive primary health care programs and services. Provide quality care coordination for referred Danila Dilba Health Service (DDHS) clients with chronic disease and complex needs to support them in accessing relevant services Working effectively with Indigenous outreach workers to engage with clients Involving the client's family / carer as appropriate. Assisting clients to adhere to treatment programs and develop chronic condition self-management skills Providing appropriate clinical care consistent with the requirements of the role (this may be in the client's home or clinic setting) Maintaining quality client medical records Employ a holistic, culturally appropriate approach to the client, drawing out cultural or social issues which may be impacting the client Is the role right for you? Qualification as a health professional (e.g. Nurse or Aboriginal and Torres Strait Islander Health Practitioner) and current registration with AHPRA Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g. diabetes, cancer, respiratory and / or cardiac disease) and relevant management strategies Current CPR certificate or the preparedness to gain one within 6 months of commencement Current driver's licence.
