Care Coordinator - Kings Cross

In line with the Wayside mission and values, the Care Coordinator is responsible for proactively supporting and empowering visitors to identify and work towards achieving their personal goals.
Your role will not be easy; however, this is a fulfilling, rewarding opportunity to play a key role in supporting and nurturing members of Wayside's community.You will have a deep understanding of the needs of individuals experiencing homelessness, particularly in relation to mental health and alcohol and other drug (AOD) issues.
Experience in case management, incident reporting, and conflict resolution is highly desirable.
Above all, you are passionate about enhancing outcomes through compassionate care, effective communication, and a commitment to excellence in case management.
You will thrive in a supportive and inclusive team environment, working from our Kings Cross office from 8.30am to 4.30pm.Key ResponsibilitiesBuild rapport with visitors and identify presenting needsConduct initial assessments, identify goals with regard for visitor strengths, and develop plans to improve visitor wellbeingRegularly review care plans to assess progress and additional needsAssist visitors to make informed decisions by providing options and reasonable advice with consideration of the presenting circumstances of the visitorCoordinate care for visitors which may include liaising with multiple social, health, housing, financial and legal services, making appropriate referrals and advocating for optimal outcomesPromote independence, self-determination and self-worth in visitors through one-to-one and community engagementCollaborate and work effectively with other programs within Wayside for the advancement of visitor care objectivesParticipate in case planning and case review meetingsParticipate in interagency meetings when required and ensure Wayside offers a consistent and professional presence.
Explore opportunities to expand relationships and collaborative work with local service providersDevelop content and facilitate therapeutic groups when appropriateOperate with regard to trauma informed principlesOperate within a harm minimisation framework when working with visitors engaged in high-risk behavioursRecord accurate and timely data and information and effectively use available systemsDevelop and maintain detailed visitor files including needs assessments, care plans, brokerage and case notesEnsure all reports and administrative tasks are completed to a professional standardAbout youUndergraduate qualifications in Social Work/other relevant disciplines or substantial experience working in Community ServicesExperience working with at-risk populations and understanding of related issues, both practical and systematic (including but not limited to Juvenile Justice, CALD, trauma, homelessness, mental health, alcohol and other drugs)High level interpersonal communication and problem solving skillsAbility to work as part of a team with a commitment to sharing knowledge and expertise and supporting Wayside cultureInterest in working in a not-for-profit and alignment to Wayside's mission and valuesWhat's in it for you?3 additional days of Flex Leave17.5% annual leave loading$15,900 of your annual salary tax free through generous NFP salary packagingPaid Cultural and Ceremonial LeaveComprehensive Employee Assistance Program accessAccess to external clinical supervisionTo apply, please follow the link.
