As part of our mission, we are seeking an experienced Intake and Care Coordinator to join our team in Tullamarine.Role Overview:The Intake and Care Coordinator will be responsible for managing the intake process for new clients, overseeing ongoing client care, and coordinating with support workers to ensure the delivery of exceptional services.
This role requires strong managerial experience within the NDIS and disability sector, as well as excellent interpersonal and organisational skills.Key Responsibilities:Client Intake and Care Management:Oversee the intake process for new clients, including assessments and care planning.Develop and maintain individualised care plans in collaboration with clients and their families.Act as the primary point of contact for clients, ensuring their needs and goals are met effectively.Support Worker Coordination:Manage and support a team of support workers, ensuring they have the resources and guidance needed to provide high-quality care.Match support workers to clients based on needs, preferences, and availability.Conduct regular check-ins with support workers to monitor performance and address any issues.NDIS Compliance and Reporting:Ensure all services are delivered in line with NDIS guidelines and compliance requirements.Maintain accurate records of client interactions, progress, and service delivery.Prepare reports and documentation as required by management or external stakeholders.Operational Oversight:Work closely with management to improve processes, identify opportunities for growth, and ensure smooth day-to-day operations.Address and resolve client or staff concerns promptly and professionally.Qualifications & Skills:Proven managerial experience in the NDIS or disability services sector.Comprehensive understanding of NDIS processes, guidelines, and compliance requirements.Strong leadership skills, with the ability to inspire and manage a diverse team.Exceptional communication and interpersonal skills, with the ability to build and maintain strong relationships with clients, families, and staff.Highly organised, with excellent problem-solving and time-management skills.Proficiency in using case management software, CRM systems, and Microsoft Office Suite.A relevant qualification in Disability Services, Social Work, Community Services, or a related field (highly regarded).Turkish speaking will be preferable but not mandatory.Benefits:Opportunity to make a meaningful difference in the lives of people with disabilities.Work in a supportive and collaborative team environment.Competitive salary based on experience.Full-time, stable role located in Tullamarine.Application Process:To apply, please submit your CV and a cover letter outlining your relevant experience and why you are suitable for the role to ****** more information about Avsar Disability Services, visit www.avsardisability.au.Join us in delivering exceptional care and support to the disability community!Job Types: Full-time, PermanentPay: $57,586.79 – $90,000.00 per yearSchedule:Monday to FridayEducation:High School (Year 9-11) (Preferred)Experience:Management: 1 year (Preferred)Licence/Certification:Driver Licence (Required)Work Authorisation:Australia (Required)Work Location: In person
