You will provide mentorship and guidance for a care team of PCW's and Registered Nurses within our facility. You will play a pivotal role in the quality of life that our residents experience. In this role you will: Oversee and co-ordinate the completion of baseline and required focus assessments of individual residents/client's needs and record findings within a Positive Wellbeing Model;Ensure that care plans are developed and reviewed based on the assessment findings of individual residents/client's needs, in accordance with the organisation's timelines;Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;Monitor that resident/client documentation is accurately reflective of care provisions and is in accordance with organisational requirements;Co-ordinate the care provision of external service providers;Assist in the review and maintenance of clinical programs and initiatives;Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.What you can bring to the team This role would be ideal for an experienced Registered Nurse with several years aged care experience who is looking for a new challenge. You are often recognised for your highly developed problem-solving, conflict resolution, decision-making and negotiation skills.
