The day-to-day responsibilities of this role cover:
Coordinating Commonwealth Home Support Program (CHSP) services Intake of CHSP funded clients Respond to all enquiries and feedback for CHSP services Completion of all required documentation and processes utilising Ourcare's CRM system Referrals to external providers as needed Monthly reporting Preferred skills and experience include:
Certificate IV Aged Care (or willingness to obtain) and or Case Management experience At least 3 years' experience as a Coordinator of a CHSP – HCP Program or Health and Community Program or related areas Strong team player with a flexible mindset and client service focus Flexible working hours and salary packaging is available.
Mandatory documentation, including a current driver's license, or willingness to obtain is required.
