You will provide mentorship and guidance for a care team of PCW's and Registered Nurses within our facility.
In this role you will:
Oversee and co-ordinate the completion of focus assessments of individual resident needs and record findings within a Positive Wellbeing Model;Ensure that care plans are developed and reviewed based on the assessment findings of individual resident needs;Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;Monitor that resident documentation is accurately reflective of care provisions and is in accordance with organisational requirements;Co-ordinate the care provision of external service providers;Assist in the review and maintenance of clinical programs and initiatives;Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.In this position you will be working on the floor Monday – Friday, PM shifts. As the Care Coordinator, ideally your experience and qualifications include:
Current AHPRA registration as a Registered Nurse, with min 2 years of aged care experience;Experience in managing and leading teams in the aged care sector;Strong knowledge of the aged care accreditation system and compliance;Demonstrated knowledge of AN-ACC;Experience in educating, mentoring and motivating staff;Superior communication skills both written and verbal;Be empathic, compassionate and committed to Resident Centric Model of Care.Importantly, you will be someone who closely aligns with our organisational values of: Unconditional Love, Continual Innovation, Mutual Trust, Wise Stewardship and Safety.
