We serve over 1,100 individuals across nearly 195,000 square kilometres in NSW and QLD, with a dedicated team of over 600 staff members. We are expanding our services in the Hunter Region, enhancing our commitment to quality care.
The RoleAs a Care Coordinator, you will play a vital role in delivering high-quality services through assessment, case management, and service monitoring for eligible clients, aligned with McLean Care's values and policies. Reporting to the Clinical Care Manager of Home and Community Care Services NSW, you will need:
Candidates with a Certificate IV in Individual Support and case management experience or an Enrolled nurse qualification are encouraged to apply.The ability to balance client expectations with available resources, effectively communicating these to clients.Develop and review individualised care plans based on client assessments and preferences.Excellent communication, interpersonal, and negotiation abilities.Maintain accurate records and documentation in line with organisational policies and government funded guidelines.A collaborative approach to working as part of a team.Knowledge of Commonwealth Support Programme (CHSP) is highly valued.Demonstrated experience in assessment, referral, and coordination of human services.This role requires travel to clients' homes; however, office-based tasks can be completed remotely (WFH).What We Offer Salary packaging options (up to $18,900 net) to enhance your take-home pay.Access to the Blue Light Card for retail discounts.Employee Assistance Program (EAP).Free influenza vaccinations and health initiatives.A supportive culture focused on people-centred care.If you are passionate about making a positive impact and contributing to your community, we would love to hear from you!
