About the role: The Care Coordinator is responsible for coordinating care and support that assists our clients to achieve their individual goals, maintain or increase their wellbeing and live the life they choose. The successful applicant will:
Demonstrate a proactive approach to assessment, planning, review and delivery of support options that are spirituality and wellness-focused and build on the strengths, capacity, goals and interests of clients. Coordinate, review and monitor the delivery of supports and services identified in the client plan. Creatively address barriers and encourage actions that promote a level of independence, reduce risk and ensure people remain well for as long as possible. Maintain accurate and comprehensive records, following policies and procedures, including client records. Participate in the Care Management Buddy Program and report into assigned Senior Care Manager for peer support and mentoring. Ensure adherence to all legislative and regulatory requirements. Understanding of, and commitment to, person-centred and strengths-based practices. Knowledge and understanding of the disability and aged care sectors. Demonstrated experience in case management or support coordination. Demonstrated experience managing and coordinating individual care needs. Superior customer service and engagement skills, with the ability to continuously build a strong client base and develop solid relationships with existing and potential clients. Demonstrated experience using Microsoft Office Suite and Client Management Systems. Demonstrated ability to work collaboratively and build strong and productive working relationships both internally and externally. Relevant tertiary qualifications and/or demonstrated experience in Community Services. Excellent organisational and time management skills, with a high attention to detail and ability to establish priorities. Excellent communication skills, both verbal and written. Current unrestricted NSW Driver's License. A current Working with Children Check and NDIS Worker Screening Check. Experience in the Social and Community Services sector.
