You will provide mentorship and guidance for a care team of PCW's and Registered Nurses within our facility.In this role you will:Oversee and co-ordinate the completion of focus assessments of individual resident needs and record findings within a Positive Wellbeing Model;Ensure that care plans are developed and reviewed based on the assessment findings of individual resident needs;Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;Monitor that resident documentation is accurately reflective of care provisions and is in accordance with organisational requirements;Co-ordinate the care provision of external service providers;Assist in the review and maintenance of clinical programs and initiatives;Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.In this position you will be working on the floor Monday - Friday, PM shifts.What you can bring to the teamThis role would be ideal for an experienced Registered Nurse with several years aged care experience who is looking for a new challenge.
