The Care Coordinator serves as a conduit between the patient, nominated carer, treating clinical team, and the Psychiatrist, providing clinical care and coordination for the patients allocated to their VMO(s) across all service settings. As an integral member of the hospital management team, responsibilities include:
Assessment and screening Collection of collateral information from referrers and family Goal setting and discharge planning Education sessions with the patient and their nominated carer Discharge planning and outcome studies Join us as we prepare for our hospital opening in January 2025, within a supportive team of health professionals, and become a part of a successful and growing organisation.
Our ideal candidate will meet the following criteria: Registration as a Registered Nurse with AHPRA and a current annual practicing certificate Postgraduate qualifications in mental health Minimum of five years clinical experience in a mental health setting Problem-solving ability and triage skills Ability to coordinate and manage multiple concurrent projects Effective time management Excellent communication and liaison skills Strong documentation and report writing skills Ability to remain calm and professional, and to work collaboratively with consumers, carers, staff, hospital accredited professionals, and members of the public You will be a self-starter, love to work autonomously, and always display a high level of customer service and professionalism.
