We aim to provide a life-changing experience for both the people who access our services and those who work with us.The Care Coordinator serves as a conduit between the patient, nominated carer, treating clinical team, and the Psychiatrist, providing clinical care and coordination for the patients allocated to their VMO(s) across all service settings. As an integral member of the hospital management team, responsibilities include:Assessment and screeningCollection of collateral information from referrers and familyGoal setting and discharge planningEducation sessions with the patient and their nominated carerDischarge planning and outcome studiesJoin us as we prepare for our hospital opening in January 2025, within a supportive team of health professionals, and become a part of a successful and growing organisation.Our ideal candidate will meet the following criteria:Registration as a Registered Nurse with AHPRA and a current annual practicing certificatePostgraduate qualifications in mental healthMinimum of five years clinical experience in a mental health settingProblem-solving ability and triage skillsAbility to coordinate and manage multiple concurrent projectsEffective time managementExcellent communication and liaison skillsStrong documentation and report writing skillsAbility to remain calm and professional, and to work collaboratively with consumers, carers, staff, hospital accredited professionals, and members of the publicYou will be a self-starter, love to work autonomously, and always display a high level of customer service and professionalism.
