In this full-time position reporting directly to the Clinical Care Manager, you will be at the forefront of our home care operation, wielding your expertise and dedication to ensure its unparalleled success. The Care Coordinator will be responsible for: Building strong connections with clients and their families Assigning staff to clients based on staff qualifications and availability, as well as client preferences Organising support for clients and monitoring and reporting on their progress Referring clients to agencies that can provide additional help Assessing client needs and resources for health, welfare and other facilities and services Responding promptly to emails, enquiries and requests from clients, staff and management Maintaining accurate records of incidents and feedback as required Reorganising work priorities to meet the needs of a fast-paced environment Supporting all clients to meet their goals Conducting Client Care Assessments and Care Planning Providing clinical guidance to other team members for client management Discussing and promoting Right at Home's services with medical professionals and referral sources Ability to work in a fast-paced, dynamic environment Your application will include the following questions: Which of the following statements best describes your right to work in Australia?
