We are looking for an energetic and passionate case manager to join our team in assisting clients and their families to live their best life.
About the role
· Liaising with clients as well as their families on a regular basis, to assess client satisfaction and address concerns
· Have an understanding and experience in delivering services according to the Aged Care Quality Standards
· Conduct comprehensive client assessments and development of care plans to support our clients at home
· Coordinate and arrange a variety of in-home support services, including personal care, nursing care, therapy, and allied health services.
· Liaise with external service providers to ensure seamless delivery of care.
· Maintaining up to date client records
· Generating regular reports for management and quality improvement purposes
· Managing HCP budgets to ensure client care needs & activities are met
· Assist with clients' activities.
· Participate in project work as directed by the Home Care Manager
· Basic level of knowledge on expenses claims via Google form
· Office expenses process/ Basic Invoice Data entry in Xero

Apply if you are:
· Qualification in Cert IV Aging Support / Case Management / Allied Health or Nursing with relevant primary care experience.
· Demonstrated experience in caring for older people.
· Excellent problem-solving skills for complex clients and appropriate decisions making capacity.
· Assessment and care planning skills
· Excellent communication skills
· A team player
· IT skills – MS Office basic, intermediate level of skill across all packages or Office, Word and Excel
· Knowledge of Home Care Packages and Aged Care Quality Standards
· Fluent in English and Cantonese or Korean.
