In this full-time position reporting directly to the Clinical Care Manager, you will be at the forefront of our home care operation, wielding your expertise and dedication to ensure its unparalleled success.The Care Coordinator will be responsible for:Building strong connections with clients and their familiesAssigning staff to clients based on staff qualifications and availability, as well as client preferencesOrganising support to clients with disabilities and monitoring and reporting on their progressReferring clients to agencies that can provide additional helpAssessing client needs and resources for health, welfare and other facilities and servicesResponding promptly to emails, enquiries and requests from clients, staff and managementMaintaining accurate records of incidents and feedback as requiredRe-organising work priorities to meet the needs of a fast-paced environmentSupporting all clients to meet their goalsConducting Client Care Assessments and Care PlanningProviding clinical guidance to other team members for client managementDiscussing and promoting Right at Home's services with medical professionals and referral sourcesAbility to work in a fast-paced, dynamic environmentQualificationsQualified NurseCommitment to a culture of teamwork, compassion, and service excellenceStrategic thinking abilitiesUnderstanding of the dynamics of a commercial operationDemonstrated leadership skillsStrong judgment, quick decision-making, and effective staff managementSome understanding of education and accreditation standards, Continuous Improvement, Occupational Health and Safety (OHS), and risk managementCurrent National Police ClearanceAHPRA RegistrationPreferred Skills
