We are looking for a care co-ordinator to complement our dynamic team at community care services.

Position Title: Care Coordinator
Classification: Community Development Worker XV10 – ON14
Salary per hour (exc.
super): $33.51 - $45.76
Hours per fortnight: Multiple positions available up to 1.42 FTE
Employment Status: Ongoing

The Community Services Team comprises Aged Care Assessment Services, Community Allied Health Services, Community Care Services, Carer Support Services, and Community Nursing and Home Care Services.
Community Care Services conducts a high-quality case management service across the Loddon Mallee region, supporting people who are frail aged and younger people with a disability and their carers to live independently in the community.
The program provides:
Support coordination for people with disability (under National Disability Insurance Scheme (NDIS)) Care coordination for Home Care Package (HCP) consumers Short Term Restorative Care Program Home and Community Care Program for Younger People (HACCPYP) Working with Bendigo Health Bendigo Health is a leader in regional health care, combining outstanding services with a strong education, training, and personal development culture.
Bendigo Health has around 5000 staff and a catchment area covering a quarter of the size of Victoria.
The three main campuses of Bendigo Health are in Bendigo, with many services extended to regional settings including areas such as Mildura, Echuca, Swan Hill, Kyneton, and Castlemaine.
