You will work within CHSP budgets and engage professionally with families and external stakeholders to meet the consumers best interests.
You will regularly provide outreach support meeting with consumers both in the community and in their homes.
As the Care Coordinator you will hold a Diploma in Community Services or Case Management and have extensive experience (minimum 3 years) in the Aged Care Sector.
You will have relevant experience in care coordination, care management and assessment and a demonstrated ability to provide delivery of services to the aged within their homes.
You will also have experience supervising small teams with a high level of motivation and a team orientated approach.
To be successful in this role you will have: Diploma in Community Services, Case Management and/or 3 years' experience in the Aged Care sector is essential.Relevant experience in the service planning, co-ordination and delivery of services to the frail aged in their own homes including care management skills and assessment.Experience in supervising a small team with a high level of motivation and a team orientated approach.Proven experience in supporting staff, and demonstrated ability to work independently, multitask and work under pressure and a commitment to a productive, cooperative, and friendly workplace.The ability to coordinate and support regular small group work.Knowledge and understanding of the Australian Government Department of Health Aged Care ongoing reforms, legislative changes and quality and compliance requirements; the National Aged Care Quality Standards; and the My Aged Care (MAC) portal.Proven experience in working within a Consumer Centred Environment.Experience in working with, invoicing, inputting service data and maintaining timely and organised records system.For further information on this exciting opportunity, please contact Jenny Lintern, Aged Care & NDIS Services Manager – Alice Springs .
