About the Role Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
You will be based across Concord, Camperdown and Canterbury.
The Out of Hospital Care (OHC) service incorporates the ComPacks, Safe and Supported at Home (SASH) and End of Life (EoL) programs.
The OHC service supports hospital discharges and prevents avoidable admissions by providing short and medium-term packages of non-clinical care.
The Care Coordinator role provides support to the OHC Team, managing client enquiries and completing various administrative tasks supporting ongoing client case management, ensuring the smooth functioning of the service.

Ability to follow guidelines relevant to position as well as demonstrated problem solving capacity.
Current NSW Drivers Licence (P1 and P2 acceptable).
