As our RN/EN Care Coordinator, you'll play a vital role in managing a portfolio of clients from our office.
You'll work closely with families, conduct in-home care assessments, and create personalized care plans.
This position is ideal for someone who thrives under pressure while maintaining a compassionate, client-focused approach.
Build meaningful relationships with seniors and their families, providing empathetic and patient support.Conduct in-home care assessments and craft care plans tailored to individual needs.Manage government-funded and private-pay cases, ensuring seamless service delivery.Train and guide care team members, fostering a culture of excellence.Collaborate with hospitals, clinics, and care facilities to expand our reach and impact.Represent Right at Home in community activities and committees.Maintain accurate and up-to-date documentation using our care planning technology.Minimum Requirements: A genuine passion for helping others and a commitment to quality care.National Police Check.Current First Aid and CPR certification.Reliable transportation and comprehensive car insurance.If EN or RN: AHPRA registration and Professional Indemnity Insurance.Preferred (but not required): Working with Children Check Certificate.Willingness to gain NDIS Screening.What Sets You Apart:
