Right at Home – Sydney Lower North Shore provides high quality in-home care to the elderly and those living with a disability who require assistance in their home. We require a passionate and dedicated Care Coordinator to join our office to assist in the performance of the co-ordination of in-home care. Due to our demographics, we require a Care Coordinator who is a Registered Nurse with English & Chinese speaking skills. This will be a part-time position with expected growth over time.Responsibilities will include:Conducting Client Care Assessments and Care PlanningSupervision and management of Care StaffDiscussing and promoting Right at Home's services with potential clients and referral sourcesThis role reports to our Care Manager.QualificationsRegistered NurseNational Police CheckCurrent First Aid/CPR CertificateDrivers LicencePreferred SkillsSound understanding of the principles of My Aged Care, Aged Care Reforms, Consumer Directed CareSignificant experience and understanding in the Australian Aged Care or Disability sectorMandarin/Cantonese and English speaking skillsEmpathetic, passionate, and caringExperience in assessing and planning care needsClient focus based on their goals, preferences, and needsExperience managing staffComprehensive Assessment and Case Management experienceExperience in provision of care and services to people from culturally and linguistically diverse backgroundsCommitment to work within RAH Mission, Vision and ValuesCapacity to take initiative and to be innovativeHighly developed organisational, administrative, business, people, and financial management skills with proven ability to determine priorities, coordinate activities, and meet deadlinesCustomer service experienceVehicle with comprehensive insurance
