We require a passionate and dedicated Care Coordinator to join our office to assist in the performance of the coordination of in-home care. Due to our demographics, we require a Care Coordinator who is a Registered Nurse with English & Chinese speaking skills. This will be a part-time position with expected growth over time.
Responsibilities will include:
Conducting Client Care Assessments and Care Planning Supervision and management of Care Staff Discussing and promoting Right at Home's services with potential clients and referral sources This role reports to our Care Manager.
Qualifications Registered Nurse National Police Check Current First Aid/CPR Certificate Drivers Licence Preferred Skills Must have a Sound understanding of the principles of My Aged Care, Aged Care Reforms, Consumer Directed Care Must have Significant experience and understanding in the Australian Aged Care or Disability sector Mandarin/Cantonese and English speaking skills Must be Empathetic, passionate, and caring Must have Experience in assessing and planning care needs A client focus which is based on their goals, preferences, and needs Must Demonstrate experience managing staff Must have Comprehensive Assessment and Case Management experience Must have Experience in provision of care and services to people from culturally and linguistically diverse backgrounds Must have a Commitment to work within RAH Mission, Vision and Values Must have the Capacity to take initiative and to be innovative Must have highly developed organisational, administrative, business, people, and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines Must have Customer service experience Must have a vehicle with comprehensive insurance
