Short Description Based at Park Holme initially with opportunity to work at other locations after training Permanent Full Time Opportunity Six positions available The opportunity The Care Coordinator (CHSP) supports CHSP clients by ensuring the delivery of services in line with National guidelines and Silverchain policies.
Focused on providing a positive customer experience, the Care Coordinator handles client intake, referral, onboarding, and ongoing assessments.
They deliver high-quality care that promotes client independence and social connectedness, respecting each client's unique goals and preferences.

The major accountabilities of this position are: Manage intake and referral processes for new CHSP clients, including onboarding and assessments and reviews Provide excellent customer service to clients, families, and stakeholders.
Coordinate and deliver client services safely and effectively.
Respond to changing client needs and update care plans accordingly.
Maintain accurate records and ensure compliance with Aged Care standards.
Support scheduling of home support and manage 'Services In Place' in My Aged Care.
Follow organizational policies and manage incidents, hazards, and complaints.
Meet CHSP KPIs and promote a reablement approach in customer interactions.
Maintain confidentiality of client and company information.
About you To be considered for this role, you must meet the following selection criteria: At least 1 year of experience in client coordination or similar role Relevant industry experience with a strong focus on customer service in a similar or comparable sector.
Genuine passion for providing care to older people within the community.
Self-motivated with a positive and flexible attitude.
Excellent communication, analytical, problem-solving, and organizational skills.
Sound administration skills with the ability to touch type while speaking to clients on the phone using a headset.
Ability to work effectively with people from diverse backgrounds and cultures.
Strong internal relationship-building skills, including working within a multidisciplinary team.
Proven ability to meet deadlines, work under pressure, and manage multiple priorities.
Capacity to work independently and proactively, identifying issues and proposing solutions.
Eagerness to network and promote services.
