More information can be found on the Uniting AgeWell website - www.unitingagewell.org About the role: Based in our Swan Hill office supporting our Home Care Program; the Care Advisor will act as the key contact person and advocate for clients in facilitating the identification of their goals and development of their care plans. This full time role offers an exciting opportunity to join our dynamic team.  Your primary focus will centre around the below core functions: Undertaking initial assessments of prospective clients and assist in making recommendations on capacity  Assess, monitor, review, reassess the needs of clients within a holistic framework using a restorative and wellness approach  Promote a harmonious team approach by acting as a mentor and supervisor to the Home Care Workers to ensure delivery of community services Facilitate effective client pathway support through case discussions and case conferences, review of feedback including incidents, hazards and handovers and monitoring of quality indicators  Conduct annual reviews with clients, including, revising their care plan, goals, services and budgets to support our clients in living life to fullest About you: You will have a knack for overcoming obstacles, finding solutions and adapt well to change.
