The core function of Carbal Medical Services is to provide a range of health services to Aboriginal & Torres Strait Islander people through two medical practices and multiple community programs.
Carbal covers the regions of Darling Downs, Southern Downs and Goondiwindi.

This role requires the Support Coordinator to be able to: Be part of a multidisciplinary team Provide person centred support, understanding a participant's right to choice and control.
Have the proven ability to work independently and possess great communication skills.
Support NDIS participants to build an understanding of their plan and implement it to achieve goals.
Assist engagement with NDIS providers, community, mainstream and government services.
Be driven, motivated and have a passion to assist people to live their best life.
Work closely with GPs, Aboriginal Health Workers, other health professionals, other NDIS Providers and informal supports.
Possess skills for report writing, documentation, reporting and multi-tasking.
Build and maintain respectful and meaningful community relationships.
Requirements Integrity, honesty, compassion, and a strong work ethic Have an understanding or the ability to rapidly acquire an understanding of the NDIS.
You will have a demonstrated understanding of how social, emotional, and physical wellbeing can impact on a person's life and relationships.
You must have a C Class driver's licence.
Hold or be eligible to obtain a Worker Screening and Working with Children check You will have a demonstrated ability to work as part of a team and independently.
The ability to keep an accurate record of supports provided.
Possess the skills to multitask and prioritise caseload demands.
Identify as a person of Aboriginal or Torres Strait Islander descent Previous experience in the NFP, mental health or disability sectors The ability to understand and implement individual support plans Experience in working effectively with Aboriginal and Torres Strait Islander people and an understanding of Aboriginal and Torres Strait Islander Culture.
