Calvary Community Care is seeking a Home Care Package, STRC, CHSP, DVA, and TCP Intake Officer. This new and dynamic role will involve working with care managers of the Home Care Package, CHSP, STRC team and all relevant parties. You will be the first point of contact for our Home Care Package, CHSP, TCP, DVA, and STRC clients when they wish to sign up to Calvary Community Care. Your role will focus on a holistic approach to help them develop their care plan and services within their funding.
Your main duties will include:
Initial contact and relationship development with new Home Care Package, CHSP, STRC, TCP, and DVA clients Completion of new client comprehensive assessments and paperwork, especially for CHSP, STRC, DVA, and TCP clients. For HCP clients, you will work with care managers to support the intake process. Development of a goal-centered approach to their care needs About you
Driver's License Certificate 3 in Aged Care Experience as an Intake Officer or in Administration NDIS clearance Police check Why work for Calvary?
