Coordinator – Healthy Hub Connect Program
Contract Full-time or part-time flexible 2-year contract position This position is based at the Coffs Harbour office About the role
As Coordinator, you will play a key role and be responsible for leading the Healthy Hubs Connect (HHC) model of care from the Coffs Harbour office.
Your duties will include:
Engaging directly with clients in a brief capacity, as well as overseeing their care journey both internally and externally to the relevant hub. Implementing and overseeing processes at the hub to promote collaborative care for service users. Co-implementing a model of brief intervention after-care for targeted HHC customers. Monitoring outcomes and demand associated with the HHC model of care, and collaborating with HHC partners to coordinate impactful collaborative responses to local needs. Working in partnership with the local HHC lived experience workforce to build community-based responses at the Hub. As the Coordinator – Healthy Hubs Connect, we are looking for someone with extensive experience working within a mental health setting supporting young people and families, and a commitment to delivering a person-centred approach.
An ability to work collaboratively and professionally with other services and professionals across a range of life areas and needs, as well as a knowledge of the mental health sector and services offered to the community to support those with complex needs will be essential to succeed in this role.
