As we continue to grow, several exciting opportunities are available for reliable and motivated Physiotherapist (Senior positions are available) to join our Brightwater At Home team working in our Community Aged Care programs.
There is the capacity for full-time/part-time work opportunities, working Monday to Friday with our clients in the community. The ideal candidate is available to work a minimum of 3 shifts per week.
You will have the ability of self-rostering with flexibility in how you work your contracted hours. Working with clients with TCC, CHSP, and HCP funding. You will work with a mix of clients receiving either Home Care Package or Transitional Care Community package.
Transition care provides short-term care to optimise the functioning and independence of older people after a hospital stay. Transition care is goal-oriented, time-limited, and therapy-focused. The programme seeks to enable older people, where possible, to return home after a hospital stay rather than prematurely enter residential aged care.
You will be working as part of a multidisciplinary team delivering the program with support from dedicated client coordinators, physiotherapists, occupational therapists, speech pathologists, dietitians, social workers, nurses, and community support workers.

To be successful for the role, you will possess the following attributes: Registered as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA). Strong commitment to providing, and the capability to deliver, safe high-quality care. Demonstrated community experience and competency as a PT to assess, plan, implement, and evaluate client care. Excellent written, verbal, and interpersonal communication skills in various environments with a diverse range of people. Strong person-centred skills in communicating and engaging with clients with high support needs including cognitive impairment, brain injury, and dementia. Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve, and meet deadlines. Working knowledge of occupational safety and health legislation; Aged Care Act (1997); and/or Disability Services Commission Standards (1988). First Aid Certificate less than 3 years old (desirable; essential if providing services for Brightwater At Home clients). Current Western Australian 'C' Class Driver's Licence. A safe, registered, and suitably insured motor vehicle able to be used for work purposes. Satisfactory National Police Certificate obtained within the last 12 months.
