We are searching for a pro-active and motivated Specialist Support Coordinator to join our team.
About the position:
The position will be working with participants with complex care and medical needs across the life span, to identify and address barriers to implement their NDIS plan.
The Specialist Support Coordinator is required to build relationships with participants, their family, disability services organisations, clinical staff and various government departments, including health, education, communities and justice.
Brighter Futures Consulting encourages a collaborative approach by stakeholders to ensure supports are delivered in a person-centred and timely manner.
This position assists with building a foundation for best practice care, staff training, support, and intervention for participants, ultimately achieving positive and sustainable outcomes following a period of crisis.
Key Requirements:
A bachelor's degree in Social Work, Occupational Therapy, Physiotherapy, Psychology, Nursing or other Allied Health.
Demonstrated well-developed oral, written and interpersonal communication skills.
Demonstrated ability to work well independently and cooperatively as a member of a team.
Established ability to listen actively and demonstrate empathy.
Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.
Working with Children card; NDIS Worker Screening; Police check (within 6 months); current Drivers Licence and roadworthy vehicle.
Computer literacy – efficiency in Microsoft Office.
