About UsApunipima Cape York Health Council is a community controlled Aboriginal and Torres Strait Islander Health Service that provides culturally appropriate, family centred, primary health care services to the communities and people of Cape York.
About the RoleAs Aboriginal and/or Torres Strait Islander Health Practitioner - Rheumatic Heart Disease (RHD) you will upskill and deliver comprehensive rheumatic heart disease primary health care services within a multidisciplinary team, consistent with the Apunipima Way and Model of Care.
This position acts as a role model in the provision of holistic care, which aims to:

Prevent and reduce the incidence of Acute Rheumatic Fever (ARF) and Rheumatic Heart Disease (RHD);
Aid in the early detection of Rheumatic Heart Disease;
Improve the management of Acute Rheumatic Fever and Rheumatic Health Disease in Aboriginal and Torres Strait Islander people living in Cape York Communities.
The position is fulltime, fixed term with an end date of 30 June 2025.
This position is identified where, under section 25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.
About YouTo be successful in this role you will have:

More than four years' experience as a health worker.
Demonstrated experience as a Health Worker within a clinical practice environment, with a sound understanding of integrated client care.
Demonstrated experience working collaboratively as part of a multidisciplinary team to facilitate a quality, comprehensive primary health care service.
Demonstrated experience performing clinical and health education in a primary healthcare setting.
Demonstrated ability to support the development of health workers and less experienced Aboriginal and Torres Strait Islander Health Practitioners
Demonstrated competency in the delivery of comprehensive primary health care services.
Ability to apply a high level of professional judgement, knowledge, and skills in the delivery of clinical care.
Advanced interpersonal skills, including the ability to work with a wide range of clients with diverse needs.
Organisation skills, including the ability to manage priorities and meet deadlines.
Understanding of the historical and contemporary issues that impact on Aboriginal /or Torres Strait Islander people and their communities, particularly in Cape York.
Ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people in accordance with community protocols and Customs.
Demonstrated understanding of the principles of Aboriginal and/or Torres Strait Islander community control.
Must be willing and have the ability to travel in Cape York either by light aircraft or 4WD as required.
Mandatory requirements

Certificate IV or Diploma in Aboriginal and Torres Strait Islander Primary Health Care Practice
Registration as an Aboriginal and Torres Strait Islander Health Practitioner with the Australian Health Practitioners Regulation Agency (AHPRA)
Current Queensland Driver's Licence
Current Blue Card positive notice or exemption, or willing and able to obtain prior to commencement
Current AFP National Police Clearance or willing to obtain prior to commencement
