Community Nursing is a part of the broader Community Services Department based in Ararat. We provide community nursing across the catchment area of Ararat Rural City. Community nursing includes palliative care, district nursing, hospital in the home, hospital admission risk program (HARP), home care packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.
What we OfferRegional Lifestyle – explore our regionA variety of employee benefitsOngoing professional development and career opportunitiesAbout the RoleThe Healthy@Home Program aims to keep our most vulnerable patients, especially those with complex health issues, healthier at home, aiming to reduce avoidable hospitalisations and presentations to our Urgent Care Centre. Healthy@Home uses regular phone calls to patients to conduct self-rated health checks that are designed to identify deteriorating health.
The Telehealth Assistant's role is to provide regular phone calls to patients, asking questions which allows the patient to rate their health. The Telehealth Assistant enters the patient responses in real time into computer software, which generates flags when a patient's health is potentially deteriorating. This allows for the Care Co-ordinator to review the call and arrange an appropriate response. The Telehealth Assistant will work closely with Care Co-ordinators to ensure their patients get the help they need in a timely manner. Through regular phone calls, the Telehealth Assistant provides a consistent point of contact for the patient and/or their carer.
About YouThe successful applicant will have the skills to generate a positive telephone experience for the patient; the ability to engage with a wide range of people in a non-judgemental manner; and well established computer and web based literacy.
Duties involved in this position are general administrative tasks including but not exclusive to:
Perform regular phone calls to patients and build rapport with patients and their carersGenerate a positive telephone experience for the patient in a controlled amount of timeProvide a great customer service, be worldly and non-judgemental and accepting of patient lifestyle choices.Perform general clerical tasks, including scheduling and coordinating appointments, referrals and other services as required by the Care Co-ordinator.Appointment is subject to immunisation clearance (Including COVID-19) and current and satisfactory Employment Checks (Police and Working With Children Checks).
