About the role
We are seeking a values-driven Case Manager to join our team in a permanent full-time basis at our Sunrise Centre, Berrimah. The Salvation Army, Drug and Alcohol Services – Top End provides a structured 12 week residential and day program based on the principles of Cognitive Behaviour Therapy (CBT), providing individual weekly counselling, group work, skills development and recreational activities with each client allocated a Case Manager for the duration of their program. Our rehabilitation and Withdrawal Services are open to both men and women over the age of 18 years wishing to address their substance misuse issues. Salary and conditions are in accordance with the SCHADS Award, Level 4.
Key responsibilities
Effectively build relationships with clients that encourage self-determination and participation.
Maintain ongoing assessment of the client's issues, needs and circumstances from initial intake to exit.
Develop individual case plans in collaboration with the client and include a range of interventions that address the needs or goals of the client.
Provide case management support to clients in a culturally sensitive manner.
Assist clients with medication management including S4 & S8 prescribed medications and appropriately recorded.
Regularly monitor and review the progress of individual case plans and take action where needed.
Maintain accurate up to date client records and data using TSA information systems.
The ideal candidate would have
Tertiary qualification in community services or health-related field, a degree qualification is preferred.
Relevant experience in a social service environment including AOD and or Mental Health.
A current and valid Working with Children Check or equivalent.
A current NT Drivers Licence.
