About the role
The Practitioner - Children and Youth, provides early intervention and prevention services to children, young people, and their families or significant others.
This role involves delivering support, mediation, case management, advocacy, and counselling to engage at-risk individuals and help them stay connected to their families, education, training, work, and the community.
The practitioner aims to address early signs of vulnerability and build a supportive environment for positive outcomes.
This is a Permanent Full-Time position working 76 hours per fortnight based in the Toowoomba Service Region.
The role is paid in accordance with the Social and Community Services Award - Level 4 commencing at $43.07 per/hour.
Responsibilities:
Conduct comprehensive assessments of children/young people and their families to identify needs and develop tailored case plans.
Provide case management, mediation, advocacy, and individual support through referrals, counselling, and service coordination.
Collaborate with core services such as schools, community agencies, and government departments to deliver holistic support.
Facilitate risk and safety assessments, ensuring compliance with professional standards and quality assurance guidelines.
Lead group programs and workshops to enhance social skills, mental health, and self-esteem for children/young people.
Contribute to continuous quality improvement by participating in team meetings, supervision, and professional development.
Maintain accurate records, meet service targets, and provide regular updates and reports to the Service Manager.
Actively engage in community networks, collaborate on service planning, and contribute to policy development.
Qualifications, Credentials and Other Requirements
Degree in Behavioural Science, Social Work, Psychology or similar (this is an essential requirement of the role) Current National Police Check Current Blue Card Current Queensland Driver Licence & own reliable vehicle Ability and willingness to travel within the service region.

Proven ability to engage children, young people, and families, empowering them to set and achieve positive life goals.
Strong experience working in multi-disciplinary teams to deliver seamless service provision tailored to individual needs.
Skilled in supporting children/young people and families with complex needs, including mental health, housing, and behavioural challenges.
Effective advocate for children/young people, ensuring access to appropriate, effective services aligned with their desired outcomes.
Demonstrated ability to work within a child-focused, whole-of-family approach.
Knowledgeable in relevant legislation and capable of administering and interpreting child/young person risk assessments.
