An-Acc Care Coordinator

Nursing - Aged Care (Healthcare & Medical) A great opportunity has occurred for an AN-ACC Care Coordinator to join the team at Eventide Homes (Stawell). We are a 92-bed residential aged care facility located on the edge of the world-famous Grampians National Park. The AN-ACC Care Coordinator will liaise closely with all staff in relation to the provision of services and with the Nurse in Charge in relation to providing a range of holistic, contemporary nursing and care services to residents. This role can be full or part-time. About you:

You are an experienced Registered or Enrolled Nurse with understanding of the AN-ACC funding process and be aware of AN-ACC funding changes planned. Be able to identify, during the assessment/care planning process, if residents' current AN-ACC rates reflect their current care needs. Ensure assessments, care strategies and ongoing documentation to provide the evidence to support AN-ACC claims and also AN-ACC assessments when commenced. Coordinate or deliver AN-ACC training to management, nursing and personal care employees. You are a role-model in best practice clinical care. You have an unrelenting ethos that those for whom we provide care are more important than ourselves.
