Join a multidisciplinary team delivering person centred, recovery orientated and trauma informed care, in accordance with National Standards for Mental Health Services.
RESPONSIBILITIES: Deliver community-based mental health care to service users through active therapeutic engagement, regular monitoring and review of mental state, response to treatments, and ongoing psychotherapeutic interventions, utilising various modalities and techniques, as well as collaboration with families, carers, and other services.Offer discipline-specific clinical expertise, guidance, and support to the team, service users, their families and carers, and other community services.Provide evidence-based, trauma-informed assessments, specialised therapies, and treatment interventions for individuals with acute and complex mental health issues, developing and implementing these interventions in the home setting in collaboration with the multidisciplinary team, families, carers, and other service providers.Deliver targeted brief interventions, support, and psychoeducation to service users, families, carers, and other service providers to help identify strengths, formulate goals, and develop and implement effective care plans.Facilitate access to a range of appropriate community and inpatient services to ensure that individuals receive timely, suitable support, including providing thorough handovers and coordinating safe and effective transfer or discharge processes.Complete all necessary clinical documentation, including initial assessments and risk assessments, care transfers, relevant clinical correspondence, and data collection, within specified timeframes, primarily using electronic records systems.MANDATORY QUALIFICATIONS AND EXPERIENCE: Appropriate Degree or equivalent qualification and registration with relevant professional organisation.Working with Vulnerable People Check must be obtained prior to commencement.A minimum of two years' experience in a clinical area directly applicable to the Child and Adolescent, Adult Community, Inpatient and Extended Treatment and Older Persons Mental Health Service area.CONTACT: Catherine to discuss this role further or inquire about our other positions available at ******
