The Role: The Allied Health Complex Care Coordination (CCC) team is an integral, highly valued team of multi-disciplinary allied health professionals, applying a collaborative transdisciplinary approach to achieve innovative solutions to complex issues surrounding patients.
We are seeking an experienced allied health professional to join our team based at Launceston General Hospital, to work alongside nursing, medical and allied health teams, and in close collaboration with external service providers, to support the right place right care patient journey and quality outcomes for our patients.
The successful applicant will provide expert clinical advice on care management pathways, systems navigation, discharge planning and hospital avoidance for patients with exceptional needs.
The Service: Please refer below to the Function Statement for information specific to this role. Details of appointment: Fixed term full time, day worker position, working up to 76 hours per fortnight for a period of 12 months, commencing as soon as possible.
Salary: $120,114 to $130,181 per annum.
Our Employer 11.5% superannuation contribution is on top of this amount.
Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement 2022You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
In addition, the following allowances calculated on the salaried incremental point may be available:
