Allied Health Complex Care Coordinator (521109)Department of HealthHospitals & Primary Care - Hospitals NthLaunceston General HospitalApplications must be submitted by Friday 29 November, 2024 11:55 PM AEST.Award/Classification:Allied Health Professionals Public Sector Unions Wages Agreement, Allied Health Professional, Level 4Salary: $120,114 to $130,181 per annumEmployment type:Full TimeRegion:NorthLocation:LGH Charles Street, LauncestonWe are seeking an experienced allied health professional to join our team based at Launceston General Hospital, to work alongside nursing, medical and allied health teams, and in close collaboration with external service providers, to support the right place right care patient journey and quality outcomes for our patients.The Role:The Allied Health Complex Care Coordination (CCC) team is an integral, highly valued team of multi-disciplinary allied health professionals, applying a collaborative transdisciplinary approach to achieve innovative solutions to complex issues surrounding patients.The successful applicant will provide expert clinical advice on care management pathways, systems navigation, discharge planning and hospital avoidance for patients with exceptional needs.Details of appointment:Fixed term full time, day worker position, working up to 76 hours per fortnight for a period of 12 months, commencing as soon as possible.Salary: $120,114 to $130,181 per annum.
