The Social Worker in Older Persons Mental Health Service (OPMHS) of Southern Mental Health works in a dual role between the Community and Inpatient Mental Health Teams.
Working as part of a multidisciplinary team the Social Worker contributes to consumers recovery, improved social functioning and affect a positive return to the community by delivering a range of clinical activities including mental health assessment, care planning, clinical review, psychosocial assessment, crisis management, clinical care coordination, advocacy and specialised therapy programs.
About the role: Reporting to the Team Manager this role works collaboratively with the consumer and families/carers as well as liaising closely with a network of key stake holders, these may include - veteran and community mental health services and other government and non-government organisations.
Duties for the role will include:
Care planning, clinical review, psychosocial assessment, crisis management, clinical care coordination, advocacy, and specialised therapy programs. Advocating for and negotiating on behalf of the consumer with relevant groups/organisations. Undertaking health promotion activities eg developing and implementing educational services as required. About you: An appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers. Ability to provide core social work skills including psychosocial assessment, counselling, individual and group work, relationship and family interventions, crisis intervention and brief therapy.
