Aged Care Assessment - Grampians (part of Grampians Health)  is a well-regarded aged care assessment organisation, that provides community and hospital based aged care assessment across the Grampians Region.
We are recruiting to assist in delivering administrative requirements related to the updated Single Assessment that has been implemented by the Commonwealth Government in response to the Aged Care Royal Commission (replacing the previous RAS and ACAS system).
To deliver this important service, we're recruiting Administration team members across various locations.

About the role The purpose of this role is to assist with administrative requirements related to assessment services for My Aged Care clients.
The Assessment is designed for people who may need entry-level support to help them stay at home or for more complex support related to Home Care Packages, entry into Residential Care, Respite Services and other related services.
Administration Officers work as part of a highly skilled and collaborative administrative team delivering support and guidance to the Aged Care Assessment team to deliver high quality assessments, with a focus on client experience.
Key Accountabilities  Provide administrative support, at the direction of the Team Leader, during the referral and assessment process.
Liaise with relevant staff regarding acceptance and allocation of referrals.
Assist assessment delegates with administrative tasks such as printing and sending client letters.
Coordinate and schedule appointments, meetings, and events.
Ensure accurate documentation and record-keeping.
Support the smooth operation of the assessment process.
Work as part of the Aged Care Assessment - Grampians Team to coordinate client referrals and link with appropriate service providers.
Support the delivery of coordinated and transparent services through the centralised client record and My Aged Care Gateway.
About You Minimum 1 years' administration experience within a fast paced human service working environment.
Minimum 3 months experience in aged care or aged care needs assessment preferred.
Ability to manage multiple tasks, prioritize, and maintain attention to detail Intermediate skills in the use of client and staff information Systems (e.g.
MAC, BOSSNet, IBA) highly regarded Demonstrated knowledge and/or experience in communicating with people who are aging and their families/carers.
Demonstrated understanding of the provision of community-based support services to the aged desirable.
Confident communication and interpersonal skills.
