The opportunity: We are searching for an experienced, pro-active, productive and caring individual to join our team, located in the Bowral area. In this role, you will play a pivotal part in supporting our Aged clients. Your initiative, commitment on follow-through, high level communication skills and your dedication to continuous improvement will see you thrive. This is a fast-paced role, you will need to prioritize and juggle tasks, whilst taking clients calls and managing referrals. Your day may not go as planned, but you love the challenge. There is an expectation in this role that you will need to visit clients in the Southern Highlands area.
Key Responsibilities Being able to work well both independently and as part of our team Coordination and management of Aged Client Accounts with regular reviews Main point of communication between Interchange and the client and/or their carer Follow through with incident reports, communicating with carers', creating solutions, resolutions and putting plans in place to avoid reoccurrences Arranging third party providers to deliver support according to funding requirements, including checking and signing invoices for payment. Communicating and updating our data system on all aspects of Interchange's care High level case management and case notes, ensuring documentation is up to date and accurate data is recorded in our system Regular follow-up with support staff for the development and betterment of our clients Reviewing plans and file management reviews, updating reports on a monthly or ad-hoc basis Attend Account Manager meetings, participate in exchange of information and areas of improvement Be conversant with all programs and actively promote a positive image of Interchange at all times Actively promote Interchange through networking and produce lead generation We'd like to hear from you if you have the following experience and skills:
Minimum Certificate IV in Individual Support (Aged Care or Disability) In depth knowledge of CHSP and HCP funding Proven experience in Aged Care Coordination Highly organized, with the ability to multi-task Proactive, can-do attitude with the ability to think quickly High level of verbal and written communication Ability to converse with all levels of management Ability to work effectively both independently and as part of a team Demonstrates sound judgement Ability to assist other team members when required To be successful for this role, you will need to provide a current First Aid Certificate, Working with Children Check as well as an NDIS Worker check.
