We are currently seeking a compassionate and dedicated Support Advisor to join our team and manage a diverse caseload, from Level 1 to Level 4 clients.
This role is available on a part-time or full-time basis.About the role:Delivering quality care to HCP clients to enhance their quality of life and maintain their independence.In partnership with the client, develop a goal-focused and consumer-directed Care Plan.Act as the key contact person and advocate for clients in facilitating their individualised Care Plan and services.Ensure client needs and preferences are provided within their allocated budget.Ensuring care plans are reviewed on a regular and ongoing basis in response to changes in client needs.Liaise with referring agencies, ACAT, local health providers, and community services.Providing supervision and direction to care workers.Perform any other reasonable duties as directed by Home Care Package Manager.We are looking for someone with:Previous experience in Home Care Packages / Homecare.Sound understanding of Aged Care environment including Home Care Packages and Aged Care quality standards.Excellent written documentation and assessment skills.Strong communication skills and the ability to problem solve and effectively resolve issues in a timely manner.Commitment to maintaining Work Health and Safety principles.Strong interpersonal skills to build positive relationships with clients, families, and colleagues.A valid NDIS Check, current driver's licence, and access to a registered and insured vehicle.Because we strive for the best quality care and services, we are happy to offer you:Salary sacrifice options.Employee Assistance Program.A supportive working environment with management that values and encourages ongoing learning and development.This is the opportunity you have been waiting for.
