Queensland Positive People (QPP) is seeking a job-share Aged Care Navigator to provide compassionate, client-centred support, empowering individuals to access essential aged care services. About us Queensland Positive People (QPP) is a peer-led, community-based organisation committed to improving the lives of all people living with HIV across Queensland. We provide programs and services including peer support and navigation, peer HIV/STI prevention and testing, HIV treatment and management, together with support and advocacy around stigma and discrimination, ageing and disability, mental health and community education. In short, we help people live well with HIV.
About the role The Care Finder Program is a part of a consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that targets people who have one or more reasons for requiring intensive support to interact with My Aged Care, access aged care services and/or other relevant supports in the community.
As an Aged Care Navigator , this role is a job-share position, working alongside an existing employee who will work two days per week. You will work within QPP's LIFE+ team, as well as with the Care Finder teams at MICAH and Communify, to advocate for and support individuals, empowering them to access appropriate resources, opportunities, and aged care services within the community. The primary demographic is to work with people living with HIV and LGBTIQ+ identifying people.

About you We are seeking candidates with the following skills & experience:
Tertiary qualification in human services or relevant experience in health, aged care or social services. Demonstrated understanding and sensitivity of historical and contemporary HIV and support issues relating to PLHIV as they age. Demonstrated understanding of the Aged Care Sector and support services, including an understanding of navigation/access issues, enablers, and barriers specific to the care finder target group. Demonstrated understanding of assertive outreach and case management principles. Demonstrated high level of interpersonal communication skills including a commitment to self-reflective processes, ethical, and collaborative decision-making processes and to transparent and accountable communication styles. Excellent personal work habits, including time management, administrative, planning and reporting skills; and demonstrated ability to work independently. Ability to work remotely and seek supervision effectively. Maintain exceptionally high standards in the protection and promotion of client confidentiality/privacy. Connection and alignment with QPP's values; diversity, equity, quality, freedom, empathy and belonging. Demonstrated ability to communicate, engage and build relationships with varied staff, stakeholders, community members and professional bodies while maintaining professional boundaries. Demonstrated competency in the use of Microsoft Office, including Word, Excel, and Outlook. Provide evidence of a tertiary qualification in human services or relevant experience in health, aged care or social services. Hold a current QLD Working with Children Check (Blue Card). Provide a current Disability Screening Check (Yellow Card) or evidence that a check has been applied for. Provide a National Police Check of 6 months currency or evidence that a check has been applied for. Provide a valid Driver's license with no restrictions. NB: Under current 'no card, no start' legislation, QPP will not consider applications where no current blue card is provided.
